Request Medical Records

FOR PROVIDERS

To obtain a copy of your patient's pror medical records from Dr. J. Harlan Meyer, please fax a signed records request to 808-524-0402. Make sure to include the patient's full name and date of birth. Make sure to include the requesting provider's address, contact phone number and email address

FOR PATIENTS

To obtain a copy of your personal medical records from Dr. J. Harlan Meyer, please do one of the following:

1. Fax your request to 808-524-0402. Make sure to include your full name, date of birth, your contact phone number and your email address

OR

2. Call 808-524-0400 and leave a voicemail. Make sure to include your full name, date of birth, your contact phone number and your email address

Upon receipt of your request, Dr Meyer will contact you and send you a release form. You will need to sign and return this form prior to release of your medical records.

*Please note that to cover administrative costs, personal medical records requests will incur a nominal charge
